When you get insurance, you should receive a card, and directions to a website. On the front page, there's typically a "Find a Provider" (Provider is insurance speak for Doctor, but sometimes could be a PT facility, or Hospital.)
Follow the prompts, and boom! You get a list of what you're looking for.
